iPad Lending Program in Florida: Library iPads

Mayo Clinic employees and students who are located at the Florida campus may borrow iPads from their campus library.

Eligibility

Mayo Clinic employees or students registered in the Mayo College of Medicine and are located on the Jacksonville campus may borrow an iPad (2018, 6th generation). They are loaned on a first-come, first-served basis. Borrowers must complete a borrower's agreement accepting responsibility for any loss or damage to the iPad and the cost of replacement. Only the person requesting the iPad can check it out from the library-- do not send administrative assistants or other proxies to pick up the iPad. The device will not be sent through intraclinic mail. You'll need your Mayo ID badge to check it out.

* Please plan accordingly - the check out process will take at least 30 minutes.

iPads are loaned for a 2-week period and they can be renewed twice. Once you've been notified that an iPad is available, you'll have 3 days in which to pick it up. If you don't pick it up within that timeframe, the iPad will be given to the next person on the waiting list. Overdue fees: you will be fined $10.00/day until the iPad is returned. 

For Florida borrowers:  iPads must be picked up and returned during the timeframe of Monday – Friday, 8:00 a.m. – 3:30 p.m. by the borrower to the Florida Staff Medical Library- Stabile 784 North. Do NOT place iPads in the intraclinic mail.

Please note overdue charges: You will be fined $5.00/day for every day past the due date until the iPad is returned.  Please contact library staff if you have an emergency that affects your return:  904-953-2881 (FLA).

NOTE: The iPad lending program in Rochester has been retired, effective Nov. 12, 2014.

Rules of Use

Use of iPads

  • Users are responsible for making sure that the iPad does not get damaged, lost, or stolen while it is checked out to them. Failure to return iPad or any of its accessories (Smart Cover, power cord/adapter, case), or total destruction or major dents, cracks, or scratches which impair operation will result in full cost of replacement plus a replacement fee ($350 total).
  • Apps and content may be loaded on the iPad using your own accounts. When an iPad is returned, all data and settings will be wiped from the device.
  • Users are responsible for any violations of the Information Security Policies, Procedures, and Guidelines and all other applicable Mayo policies, State or Federal laws or contractual agreements.
  • The Mayo Library assumes no responsibility for any problems that the iPad may cause to any other equipment to which the iPad is connected.
  • iPad use is a privilege that can be revoked if the service is abused.

Equipment

iPad kits include an iPad (2018, 6th generation, 128GB, wi-fi only) with cover, power cord and adapter and carrying case.
