Rare and Interstitial Lung Diseases: Clinical Cases and Real-World Discussions by Claudio Sorino; ,Sergio Agati; Sonye Karen Danoff et al, 2025 Interstitial lung diseases are rare entities when evaluated individually. Considered altogether, due to their variety (more than 200 pathologies), they constitute a significant part of the clinical activity of physicians dealing with respiratory diseases. This is an extremely niche area of interest, as it involves not only idiopathic but also autoimmune and rheumatologic pathologies, as well as diseases linked to noxious professional exposures or secondary to the increasingly widespread use of drugs. To date, we lack a standardized clinical approach that is recognized as effective and efficient in diagnosing most rare respiratory and nonrespiratory diseases, except those few conditions for which a screening process has proven useful. In fact, given their “intrinsic” rarity, loosely recommending second-level tests in all patients with evidence of interstitial lung disease would be inconvenient and uneconomical. Similarly, therapeutic options are in some cases based on a limited number of reported experiences. However, recent advances such as the availability of antifibrosing drugs have allowed progress in the management of fibrosing lung diseases and raised new research interests. Given these premises, a surely correct way to improve diagnostic yield and management skills in this field is to increase culture on these pathologies, with an innovative and intelligent approach such as the one proposed by this volume. I believe it can accompany the working lives of all of us every day and should be placed on the desks of all health care professionals called to diagnose and manage respiratory diseases.
